Job Summary:

Brighter Days Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing home health team on a PRN basis. This position offers flexibility while allowing you to provide meaningful one-on-one care to patients in the comfort of their homes.

We are looking for clinicians who are reliable, organized, patient-focused, and passionate about delivering high-quality care.

Responsibilities:

• Provide skilled nursing visits in accordance with physician orders and the plan of care.
• Perform patient assessments and ongoing nursing evaluations.
• Administer medications and treatments as ordered.
• Monitor patient condition and report changes appropriately.
• Educate patients and caregivers regarding disease processes, medications, and safety.
• Accurately complete documentation in compliance with agency policies and Medicare guidelines.
• Collaborate with physicians, therapists, case managers, and interdisciplinary team members.
• Participate in case conferences as needed.

Qualifications:

• Current Arizona LPN license in good standing.
• Current CPR certification.
• Minimum of 1 year nursing experience preferred.
• Home health experience preferred but not required.
• Strong communication and organizational skills.
• Reliable transportation, valid driver’s license, and current auto insurance.
• Ability to work independently and manage time effectively.

Work Location:

• Patients seen in their homes throughout the Phoenix metro area and surrounding communities.
