We are seeking a dedicated Hospice Chaplain who feels called to support patients and families during life’s most tender and meaningful transition. In this role, you will offer presence, compassion, and spiritual guidance to individuals facing end-of-life, helping them find peace, dignity, and connection.

This is more than a position. It is sacred work.

Position Overview

As a Hospice Chaplain, you will provide spiritual care tailored to each patient’s beliefs, values, and traditions. You will serve as a trusted source of comfort and strength for patients, caregivers, and loved ones while collaborating closely with our interdisciplinary hospice team.

Your presence will help families navigate grief, reflection, forgiveness, celebration of life, and closure.

Key Responsibilities

• Complete comprehensive spiritual assessments for hospice patients

• Provide spiritual counseling, prayer, and emotional support as appropriate

• Support individuals of all faith backgrounds and belief systems

• Participate in interdisciplinary team meetings and contribute to care planning

• Ensure accurate, timely documentation in compliance with Medicare and hospice standards

• Offer bereavement support and follow-up for families

• Serve as a liaison with community clergy when requested

Qualifications

• Minimum 1 year of chaplaincy experience (hospice experience preferred)

• Understanding of hospice philosophy and Medicare spiritual care requirements

• Training or experience in grief counseling and end-of-life support

• Deep respect for cultural, spiritual, and religious diversity

• Strong communication and active listening skills

• Collaborative team mindset

• Spanish fluency is a plus
