Your Role:

• Perform in-home patient assessments and create personalized care plans.

• Administer medications, wound care, and treatments per physician orders.

• Educate patients and caregivers on condition management and independence.

• Monitor and document patient progress, reporting updates to the care team.

• Supervise LPNs and Home Health Aides to ensure quality and compliance.

• Participate in interdisciplinary meetings to coordinate holistic care.

What You Need:

• Active Registered Nurse (RN) license in Arizona.

• Minimum of one year of nursing experience (home health, hospice, or critical care preferred).

• CPR certification and valid driver’s license with reliable transportation.

• Excellent clinical judgment, communication, and time-management skills.
