Summary

Under the supervision of Value-Based Care Operations Director, the Population Health Coordinator will be responsible for collecting, analyzing, and interpreting data to support organizational population health and value-based care (VBC) initiatives. The Population Health Coordinator will work in partnership, from a care team approach, with the Value Based Care Nurse Navigator to outreach, engage and support patients in increasing compliance with preventative and disease-based services, as well as with system navigation. This position also partners with clinic staff, to serve as a point of contact for VBC related initiatives and ensure all parties are working in alignment towards the same goals. The Population Health Coordinator monitors, analyzes, and validates claims and clinical data to support the healthcare team in proactively managing identified patients. Additionally, they will review, analyze and submit relevant clinical data from patient medical records] to VBC contract payors to assist with gap closure.

Responsibilities

Creates, manages and works with large complex lists of patient data, from varying sources, to identify, analyze charts, and close care gaps based on specific quality metric criteria. This may include identifying exclusionary criteria, specific to each metric as well as submitting relevant data to update status with payors and contract entities, while ensuring that patient’s care gaps in electronic health record are up to date.

Partners with VBC Nurse Navigator to outreach, prioritize, and engage with patients regarding coordination of care, medication adherence, closures of preventative and chronic disease management gaps in care, including but not limited to assisting with scheduling, pending appropriate orders and collecting any relevant historical data or medical records.

Serve as liaison between patients and care team by collaboratively ensuring needs are met and building rapport with patients through empathetic outreach and ongoing conversations to encourage participation in preventative screenings, reduce barriers and promote continuity of care.

Utilizes specialized knowledge, judgment, and expertise to analyze data, reports and clinic/system workflows and processes to identify improvement opportunities.

Establishes and maintains positive working relationships with patients, clinic staff, providers, payers, team members, and other stakeholders, to ensure that workflows are always most reflective of current state and staff are kept in the loop regarding changes and working to promote synergy with value-based care in practice.

Communicate normal test results to patients in a timely, clear, culturally competent and compassionate manner, ensuring understanding. Facilitating conversation between patient and Nurse Navigator to address any follow up questions or patient needs that may arise.

Participates in departmental improvements, initiatives, and performs data collection for measurement of projects.

Essential:

  • HS DIPLOMA/GED

Other Information

EQUIRED: High School Diploma/GED, Medical Assistant Certification, 5 + years of healthcare related experience.

Strong communication analytical skills. Ability to organize and managed multiple and changing priorities effectively and efficiently. Proficiency in MS office (outlook, excel, word)

PREFERRED: Bi-lingual (English and Spanish). Experience with EPIC Electronic Health Records or similar electronic record system. Experience with population health quality measures to include HEDIS, as well as familiarity with CPT charges and ICD-10 diagnosis codes.
