Your Role: Home Health Occupational Therapist

You will provide one-on-one care in patients’ homes, focusing on improving functional independence, safety, and quality of life.

Key Responsibilities

• Conduct in-home evaluations, treatments, and reassessments
• Develop individualized plans of care focused on ADLs and functional independence
• Address fall risk, home safety, and adaptive strategies
• Educate patients and caregivers on safe techniques and equipment use
• Collaborate with interdisciplinary team members
• Complete timely and accurate documentation

Qualifications

• Active Arizona Occupational Therapist license
• CPR certification (AHA)
• Minimum 1 year of clinical experience (home health preferred)
• Strong clinical judgment and time management
• Reliable transportation, valid driver’s license, and insurance
• Strong communication and ability to work independently
