MSW - Masters Social Worker
Job Title: Master Social Worker (MSW) – Home Healthcare
Location: Phoenix, AZ
Overview:
Join our compassionate team at Sun Valley Home Healthcare as a MSW in home healthcare. You will provide essential psychosocial support to patients and families, ensuring holistic care in the comfort of their homes.
Key Responsibilities:
- Conduct comprehensive assessments to identify emotional, social, and environmental factors affecting patient health.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Provide counseling and support to patients and families coping with illness, injury, or end-of-life issues.
- Connect patients with community resources and services to enhance their well-being.
- Maintain accurate and timely documentation in compliance with HIPAA and agency policies.
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited program.
- Current state licensure.
- Minimum of 1 year of experience in a healthcare or home health setting.
- Valid driver’s license and reliable transportation.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a caseload effectively.
