Full Time:Mon-Fr 2p-10pm (38-40hrs) (Must be over 21 and a driver)

Reports to: Group Home Manager

Place of Duty: Group Homes
Job purpose:
To plan and carry out direct care of individuals with Developmental Disabilities Adult/Children.

Key Responsibilities:

1. Monitor and ensure the health and safety of all individuals living in the home.
2. Administer medications as prescribed by a medical professional.
3. Prepare all necessary meals (breakfast, lunch, dinner, snacks etc.).
4. Transport individual to appointments, visits, and activities/community events.
5. Implement HAB objectives/goals as agreed upon by the ISP team.
6. When applicable implement behavior building plans.
7. Actively interact with individuals in the home throughout your shift.
8. Complete all required Documentation before leaving shift.
9. Assist with preparing monthly activity calendar and weekly menus.
10. Ensure the home is clean and in good repair.
11. Other Duties as assigned.

Qualifications:

1. Clear criminal and central registry background check (Required)

2. At least 21 years of age

3. Valid driver's license with a clean driving record (If applicable)

4. Employee also agrees to maintain all required DDD certifications and a valid fingerprint card throughout the duration of their employment with Sunny Field Residential Homes LLC.

5. Proficient with the English language: written and verbal

Education and Experience:

High School Diploma, and a minimum of one year of experience in the DDD or Behavioral Health Field.
Must apply for a Fingerprint Clearance Card within 7 days of hire.

Must have Article 9, Prevention and Support, CPR, and First Aid certifications within 90 days of hire.
