Position Details:
Full-time opportunity: Friday, Saturday, Sunday OR Mon, Tues, Wed, Thurs.
Provide in-home visits to patients in need of skilled nursing care
Deliver professional, compassionate, and reliable care
Collaborate with patients, families, and care teams to ensure the best outcomes
Qualifications:
Active LPN license in [State]
Previous experience in home health or patient care preferred, but not required
Strong communication and documentation skills
Computer literate
Dependable, compassionate, and patient-focused
