Schedule: Friday 5:00 PM – Monday 8:30 AM

We’re seeking a dedicated Weekend On-Call Licensed Practical Nurse (LPN) to join our compassionate team. In this role, you’ll be the calm and caring presence patients and families depend on after hours, ensuring that their needs are met promptly and with kindness.

Your Impact:

As the On-Call Hospice LPN , you will:

• Respond promptly to patient and family needs during scheduled on-call hours.

• Provide skilled nursing care, symptom management, and emotional support.

• Collaborate with on-call RNs, physicians, and the interdisciplinary team as needed.

• Educate families on hospice care, medications, and what to expect.

• Ensure continuity of care through timely, accurate documentation and effective communication with the weekday clinical team.

What You’ll Bring:

• Current LPN license in good standing (required)

• Minimum 1 year of hospice or home health experience preferred

• Strong clinical assessment and problem-solving skills

• Excellent communication and interpersonal abilities

• A true commitment to compassionate, patient-centered care

• Reliable transportation and valid driver’s license
