We are seeking a Home Health Care Worker to become a part of our team! You will provide routine healthcare to patients at the patient's home.
Responsibilities:
- Provide routine individualized care to persons with disabilities
- Monitor and report changes in patient health status
- Provide personal care, such as bathing and dressing patients
- Provide specialized care that focuses on non-medical services for members
- Record notes on me member progress on daily goals and the results
- Teach family members ways to care for patient
Qualifications:
- Experience in hospice, home health, or other related field service
- Must have reliable transportation and clean MVR
- Positive and patient demeanor
- Ability to build rapport with patients
- Ability to handle physical workload
- Excellent written and verbal communication skills
- Quality Customer service to members
Requirements :
- Must be have or be willing to obtain a fingerprint clearance card
- Stated Required Training must be completed within 30 days of hire
- Must be 18 years of age and older to be considered for employment
- Background is required for this role
