Your Role

• Perform in-home evaluations

• Develop and implement tailored treatment plans

• Help patients reach mobility and functional goals

• Educate families and caregivers to support recovery

• Collaborate with an interdisciplinary care team

• Document accurately with AI-enhanced tools

What You Bring

• Active Arizona PT license

• Home health experience

• Current CPR certification

• Excellent communication and clinical skills

• Reliable vehicle and valid driver's license

• Passion for patient-centered care
