Nurse Practitioner or Physician Assistant - Primary Care Functional Medicine

Responsibilities and Duties:

• Deliver direct patient care through office visits, telehealth, and occasional same-day appointments.
• Conduct comprehensive assessments, order and interpret labs/imaging, and diagnose both acute and chronic conditions.
• Develop personalized treatment plans that incorporate lifestyle changes, supplements, hormone therapies, and prescriptions.
• Educate and empower patients on preventive health and functional medicine principles.
• Collaborate with internal health coaches, nutritionists, mental health providers, and other clinical support staff.
• Ensure timely and compliant documentation of visits in the EHR (NextGen).
• Participate in team meetings, quality improvement initiatives, and training sessions.
• Contribute to a positive, team-oriented clinic environment.

Required Qualifications:

• Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain).
• Board certification in Family Medicine, Internal Medicine, or a relevant specialty.
• At least two years of clinical experience is preferred; experience in functional or integrative medicine is a plus.
• A strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Comfortable working in a fast-paced, collaborative environment.
• Proficiency with electronic health records; experience with NextGen is a plus.
