Nurse Diabetic Educator (RN/CDE/CDCES)

Position Overview:
We are seeking a compassionate and motivated Nurse Diabetic Educator to provide individualized diabetes education, medication guidance, and ongoing support for patients managing complex metabolic conditions. This role works closely with our providers to improve A1C outcomes, enhance patient understanding, and reduce complications through evidence-based education and lifestyle interventions.
The RN will play a vital role in supporting the operations of our Diabetic Infusion Therapy Program. Under the supervision of the Nurse Practitioner Program Manager, the RN will provide direct patient care, assist with treatment administration, and contribute to the overall success of the program.
Key Responsibilities:
- Provide individualized diabetes self-management education (DSME).
- Run Physiologic Insulin Resensitiation infusion.
- Educate patients on insulin management (basal/bolus), oral medications, and injectables.
- Review and interpret CGM data (Dexcom, Libre, etc.).
- Assist with insulin titration protocols under provider supervision.
- Develop personalized nutrition and lifestyle plans.
- Educate on carbohydrate counting, label reading, and glucose monitoring.
- Support patients with insulin pumps and continuous glucose monitoring systems.
- Monitor and track patient A1C trends and outcomes.
- Document education sessions and care plans in EMR.
- Collaborate with providers, MAs, and care coordination team.
- Participate in chronic care management (CCM) and remote patient monitoring (RPM) programs.
Qualifications:
- Active Registered Nurse (RN) license in Arizona.
- Certified Diabetes Care and Education Specialist (CDCES) preferred (or willingness to obtain).
- Minimum 2 years of clinical nursing experience.
- Experience managing insulin-dependent patients strongly preferred.
- Strong patient education and motivational interviewing skills.
- Familiarity with CGM interpretation and diabetes technology.
- EMR experience required.
