The Terraces of Phoenix, a distinguished and beautiful HumanGood life plan community, is hiring a full-time Social Worker. Under limited supervision, the Social Worker partners with residents, families, interdisciplinary team members, providers, managed care partners, and community agencies to support care planning, discharge or transition planning, regulatory documentation, resident rights, and overall resident well-being.
** Monday - Friday, 8:30 AM - 5:00 PM ** Pay Range: $32-$36/hr. depending on experience
To be successful in this role you would have:
- Bachelor’s degree in social work or a related field, with MSW preferred.
- Two or more years of experience in senior living, healthcare, social services, case management, discharge planning, or a related setting preferred.
- Knowledge of resident rights, care planning, community resources, and social service documentation.
- Strong communication, organization, and care coordination skills.
- Proficiency with electronic health records and Microsoft Office applications.
- State social work license/certification as required by applicable regulations.
Day in the life may include:
- Conduct psychosocial assessments and maintain required documentation.
- Coordinate care planning, discharge planning, and care transitions.
- Support residents and families through advocacy, education, and resource coordination.
- Connect residents with community services, benefits, and support programs.
- Promote resident rights, well-being, and person-centered care through interdisciplinary collaboration.
- Serving as liaison with managed care partners and communicating with residents about insurance resources and limits.
- Identifying and interacting with high-risk residents requiring social services through all ambulatory case management programs.
- Assisting Admissions Director with preparing for prospective admissions, including contact with hospital discharge planners, tours for family or responsible party, coordinating with nursing team, etc.
