Job Summary:
Physical Therapist (Outpatient) Peoria, AZ. The organization is seeking an Outpatient Physical Therapist to evaluate assigned patients, develop and implement individualized treatment plans, and support progress toward discharge goals in alignment with physician direction. The role includes completing clinical documentation, collaborating with patients and families, and serving as an educator for students, coworkers, and the broader care team. Candidates are required to bring their license on the first day.

Job Details:

• Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; CPR Certification; active/applicable license for the State where work will be performed; ability to maintain professional standards and uphold required compliance expectations.
• Preferred Qualifications: Completion of an assignment with the organization previously; successful completion of an assignment with the organization previously.
• Responsibilities: Evaluate assigned patients and complete comprehensive assessments; utilize clinical reasoning to establish treatment plans and goals focused on discharge progression; complete timely and accurate medical record documentation, including documentation requirements, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats; ensure care is provided under signed, valid physician orders and within insurance approval as indicated and necessary; delegate and direct discipline-specific therapy services for assigned patients; design functional treatment programs to support effective return to community activities; participate in student programming as assigned with appropriate supervision and learning support; create and maintain an individualized professional development plan through continuing education, self-study, and professional activities; adhere to discipline-specific State Practice Act and other regulatory requirements; support organizational performance improvement efforts and provide feedback through staff meetings, focus groups, or appropriate channels; follow clinic guidelines for personal appearance, attendance, and punctuality.
• Additional Information: Work includes following clinic guidelines related to personal appearance, attendance, and punctuality, and maintaining patient- and employee-safety awareness in areas such as safe patient handling, materials handling, infectious diseases, slips/trips/falls, and the work environment.
