Your Role and Responsibilities

• Conducting detailed patient evaluations, organizing diagnostic tests, and creating customized care plans
• Diagnosing and managing acute, episodic, or chronic medical conditions independently or in collaboration with physician partners
• Providing education to patients and their families regarding prevention, wellness, and chronic disease management
• Prescribing medications and therapeutic interventions within your practice scope
• Participating actively in chronic care management and patient follow-up
• Collaborating with a multidisciplinary team to ensure comprehensive and holistic patient care
• Maintaining accurate and thorough documentation in electronic medical records (EMR)

Educational Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree
• Graduation from an accredited Nurse Practitioner program

Certifications Required

• Active Nurse Practitioner licensure in Arizona
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or a relevant specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to achieve these within 90 days)
