POSITION SUMMARY:

• Assesses, plans and provides physical therapy services to clients in their homes in accordance with Medicare, Arizona, and Federal regulations and guidelines.
• Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNAs, and MSWs.

Qualifications:

• Professional Arizona PT license in good standing.
• Minimum of one year's experience with a preference for prior Home Health experience.
• CPR certification.
• Fingerprint clearance card.
• TB test result.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements with a good driving record.
• Demonstrates strong verbal and written communication skills.
