Your Responsibilities

• Conduct thorough patient evaluations, order diagnostic assessments, and develop tailored care plans
• Diagnose and manage acute, chronic, or episodic illnesses independently or in collaboration with physicians (based on credentialing)
• Guide patients and families in lifestyle modifications, wellness strategies, and disease management
• Prescribe medications, therapies, and interventions as per the scope of practice
• Participate in patient follow-ups and chronic disease management initiatives
• Collaborate with multidisciplinary teams to ensure comprehensive, patient-centered care
• Maintain detailed and accurate documentation within electronic medical records (EMR)

Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Licensure & Certifications

• Valid Nurse Practitioner license in Arizona
• National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or equivalent
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to obtain within 90 days)
