What You'll Be Doing:

Delivering Personalized Care: Provide compassionate medical care to individuals with CKD and/or ESRD/ESKD in their homes and virtually.
Comprehensive Assessments: Conduct thorough assessments, including annual wellness exams.
Patient Education: Counsel and educate patients and families about available benefits and programs.
Accurate Documentation: Maintain accurate and current patient records.
Care Coordination: Coordinate care with primary care providers, specialists, and ancillary services.
Evidence-Based Care: Deliver timely care that reduces hospitalizations and maximizes quality of life.
Medication Management: Prescribe medications, order tests, and collaborate with physicians.
Quality Outcomes: Improve patient quality outcomes through effective care delivery.
Preventive Monitoring: Educate patients about preventive monitoring and schedule diagnostic testing.
Educational Support: Assist patients with accessing educational videos.
Team Collaboration: Participate in integrated care team meetings.

What We're Looking For:

Bilingual (English/Spanish) proficiency (required).
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work autonomously and without direct supervision.
Access to transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management programs and medical records systems.
Excellent communication skills (verbal and written).
Sound understanding of HIPAA and OSHA regulations.
2+ years of direct patient care experience.
Managed Care/IPA/Health Plan experience (preferred).
Experience conducting annual wellness visits or similar comprehensive visits (preferred).
Work Environment:

Remote position within the state of employment.
In-home and virtual patient care.
Periodic training/meetings outside of the state may be required.
