Key Responsibilities:

• Provide one-on-one care to pediatric patients in their homes.
• Collaborate with families, support coordinators, and medical professionals to develop and implement care plans.
• Administer medications, monitor patient health, and document care activities.
• Educate families on managing their child’s health needs.
• Ensure compliance with state regulations and DNI standards.

Qualifications:

• Valid LPN or RN license in the state of Arizona.
• Current CPR certification.
• Pediatric experience preferred but not required.
• Strong communication and organizational skills.
• Compassionate, patient-focused approach to care.
