Position SummaryA growing healthcare organization is seeking an experienced Hospice RN Clinical Liaison to serve as a key clinical and community resource, helping patients, families, and healthcare providers navigate the hospice care journey. This field-based role combines clinical expertise with relationship development, education, and outreach to support appropriate hospice utilization and seamless transitions of care.
The RN Clinical Liaison partners closely with hospitals, physicians, case managers, discharge planners, skilled nursing facilities, home health agencies, and other referral sources to provide education, answer clinical questions, and facilitate timely hospice admissions. The ideal candidate is a compassionate Registered Nurse who enjoys building trusted relationships while representing hospice services with professionalism and integrity.
Education & Experience- Bachelor’s degree in Nursing or a related healthcare field preferred.
- Current, unrestricted Registered Nurse (RN) license required.
- Minimum of one (1) year of healthcare sales, marketing, or business development experience required.
- One (1) to two (2) years of hospice or palliative care marketing, liaison, or referral development experience strongly preferred.
- Clinical experience in hospice, palliative care, home health, hospital, or post-acute care settings strongly preferred.
- Demonstrated success developing and maintaining relationships with physicians, healthcare providers, community organizations, and referral partners.
- Build and maintain strong relationships with hospitals, physicians, case managers, discharge planners, skilled nursing facilities, social workers, home health agencies, and other healthcare referral sources.
- Educate patients, families, and referral partners on hospice philosophy, services, eligibility criteria, and care options in a compassionate and clinically appropriate manner.
- Serve as a clinical resource for hospice admissions, transition planning, and end-of-life care discussions.
- Develop new referral opportunities while strengthening and expanding existing professional partnerships.
- Conduct regular community outreach activities to increase awareness and appropriate utilization of hospice services.
- Collaborate with interdisciplinary clinical and operational teams to support timely, accurate, and patient-centered admissions.
- Facilitate communication between referral sources, patients, families, and internal care teams to ensure smooth transitions across the care continuum.
- Represent the organization with professionalism, empathy, and a commitment to exceptional patient and family experiences.
- Active, unencumbered Registered Nurse (RN) license in the state of practice.
- Minimum of one (1) year of healthcare sales, marketing, liaison, intake, admissions, or referral development experience.
- Strong understanding of hospice services, patient care transitions, and healthcare delivery systems.
- Proven ability to establish and maintain productive working relationships with physicians, hospital staff, case managers, discharge planners, clinical teams, and community partners.
- Excellent communication, presentation, and interpersonal skills, with the ability to educate and support patients and families during sensitive decision-making processes.
- Strong organizational, follow-up, and relationship management skills.
- Ability to work independently in a field-based environment while collaborating effectively with internal teams.
- Previous experience in hospice or palliative care marketing, clinical liaison, or business development roles.
- Experience working with hospitals, skilled nursing facilities, home health agencies, or post-acute care networks.
- Knowledge of hospice eligibility guidelines, reimbursement processes, and regulatory standards.
- Established relationships within the local healthcare community are a plus.
- Build and maintain trusted partnerships with healthcare professionals and community organizations.
- Communicate complex clinical information clearly and compassionately to patients, families, and referral sources.
- Balance clinical knowledge with business development and community outreach responsibilities.
- Work effectively in a fast-paced, field-based environment with a high degree of autonomy.
- Demonstrate professionalism, empathy, and strong problem-solving skills while supporting patients and families through the hospice journey.
- Reliable transportation required for local travel within the assigned territory.
- Valid driver’s license and current automobile insurance required.
- Willingness to travel throughout the assigned service area to meet with referral partners and patients as needed.
