We believe every patient with chronic disease deserves consistent check‑ins, follow‑up, and support. Remote Care Coordinator will perform telephonic encounters with patients on behalf of our practice partners each month. This is a 1099 Contractor position; contractors responsible for their own taxes. Esrun Health seeks Medical Assistants to work part‑time from home office as independent contractors while complying with HIPAA privacy laws. Set your own hours, not held to a daily schedule. Part‑time team members typically dedicate 20‑30 hours per week initially; time commitment increases as patient assignment increases. Flexible schedule to balance work‑life. Care Coordinator assigned patient panel based on skill and efficiency level, expected to carry a minimum of 100 patients per calendar month within first three months. Complete due diligence on 100% of assigned patients and billable encounters on 90% of patients each month unless patients unable to participate due to health conditions. Compensation Structure Productivity‑based pay structure:

  • 8.00 per completed patient encounter up to 99 encounters/month.
  • 8.50 for 100‑149 encounters/month.
  • 9.00 for 150‑199 encounters/month.
  • 10.00 for 200‑249 encounters/month.
  • 11.00 for >250 encounters/month.
Payment tier increases require 3 months consistency to achieve. $1/encounter incentive compensation for bilingual contractors equal to $3/hr applied only if hired into a bilingual position. Monthly outreach includes chart review, contact attempts (calls/texts/emails), actual call time, care coordination, and documentation/billing. Time billed in 20‑minute units of service referred to as “encounters”; each patient can be billed up to three units per month. 20‑39 min = 1 encounter; 40‑59 min = 2 encounters; >60 min = 3 encounters. Example Chart Review: 8 min Outreach Attempts: 6 min Actual Call: 11 min Care Coordination: 9 min Total Time Spent: 44 min = 2 encounters Productivity‑based position – no compensation outside billable encounters described; additional bonuses optional. No pay for onboarding. Onboarding self‑led; 3‑6 hrs, up to 14 days possible. What Your Impact Will Be Advise care according to plan and orders. Provide prevention and intervention for multiple disease conditions via motivational coaching. Develop positive interactions with patients on behalf of practices. Improve revenue by creating billable Care Management episodes. Understand healthcare goals of chronic disease management. Attend scheduled meetings (Bi‑Monthly Staff Meetings, monthly one‑on‑ones, etc.). What We Are Looking For Certified Medical Assistant. Minimum 2 years clinical experience, preferably pain management. Hands‑on EMR experience, Windows desktop, Office 365, Teams, Excel; HIPAA compliant home environment. Initiative, judgment, organization, time‑management, problem‑solving, decision‑making skills. Computer proficiency; high speed Internet; desktop or laptop (Windows or Mac). No Chromebooks or iPads. Excellent verbal, written, listening skills.
