Position SummaryWe are seeking a Lead Home Health & Hospice QA/Coding Specialist to provide expertise and leadership within our clinical quality team. This full-time role is responsible for overseeing documentation quality, ICD-10 coding accuracy, regulatory compliance, and audit readiness across home health and hospice services.
The Lead QA/Coding Specialist will serve as a subject matter expert, mentor clinical staff, identify trends, and drive process improvements to ensure documentation accurately reflects patient conditions, services provided, and regulatory requirements.
The ideal candidate has advanced knowledge of home health and hospice regulations, OASIS, ICD-10 coding, and quality assurance best practices, along with the ability to lead initiatives and support clinicians effectively.
Key ResponsibilitiesLead the review and audit of home health and hospice documentation for accuracy, completeness, and regulatory compliance
Perform and oversee ICD-10 coding in alignment with CMS, Medicare, and industry standards
Review and audit OASIS assessments, providing guidance and feedback to improve accuracy and data integrity
Identify documentation and coding trends, risks, and gaps; recommend and implement corrective actions
Serve as a resource and mentor to clinicians regarding documentation, coding, and quality standards
Collaborate with clinical leadership to support compliance initiatives and quality improvement efforts
Assist with preparation for internal audits, external surveys, and regulatory reviews
Develop and support staff education related to documentation best practices, coding accuracy, and compliance
Ensure ongoing compliance with Medicare, CMS, and state-specific home health and hospice regulations
Support continuous improvement initiatives to enhance clinical documentation quality and outcomes
Required:
Minimum of 2 years of experience in home health and/or hospice QA, coding, or clinical documentation review
Strong expertise in ICD-10 coding and home health/hospice regulatory requirements
Demonstrated experience reviewing OASIS assessments and applying quality standards
Excellent attention to detail with strong analytical and critical-thinking skills
Proven ability to communicate findings clearly and collaborate with clinical teams
Proficiency with EMR systems (e.g., WellSky, HCHB, KanTime, or similar platforms)
Preferred:
Coding certification (HCS-D, HCS-O, ICD-10, or equivalent)
Experience serving in a lead, senior, or mentorship role
