About the Role:
The Registered Nurse (RN) plays a critical role in delivering comprehensive patient care within home health and hospice settings, ensuring patients receive compassionate and skilled nursing services tailored to their individual needs. This position involves managing direct patient care, including wound management, telemetry monitoring, and patient education, to promote optimal health outcomes and quality of life. The RN serves as a patient advocate, coordinating with interdisciplinary teams to develop and implement personalized care plans that respect patient preferences and clinical requirements. Through continuous assessment and monitoring, the RN identifies changes in patient conditions and responds promptly to ensure safety and comfort. Ultimately, the role contributes significantly to the holistic well-being of patients and their families during vulnerable stages of health and end-of-life care.
Minimum Qualifications:
- Current and valid Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of clinical nursing experience, preferably in home health or hospice care.
- Basic Life Support (BLS) certification.
- Demonstrated proficiency in direct patient care and nursing documentation.
- Strong communication and interpersonal skills.
Preferred Qualifications:
- Experience with telemetry monitoring and advanced wound care techniques.
- Certification in Hospice and Palliative Nursing (CHPN) or equivalent.
- Familiarity with patient advocacy principles and ethical care standards.
- Previous experience working alongside nurse practitioners in a collaborative care model.
- Additional certifications related to home health nursing or chronic disease management.
Responsibilities:
- Provide direct nursing care to patients in home health and hospice environments, including wound care and telemetry monitoring.
- Conduct comprehensive patient assessments to develop and update individualized care plans in collaboration with healthcare teams.
- Educate patients and their families on disease management, medication administration, and health maintenance strategies.
- Advocate for patient needs and preferences, ensuring ethical and compassionate care delivery.
- Document patient progress, clinical observations, and interventions accurately and timely in compliance with regulatory standards.
- Coordinate with nurse practitioners and other healthcare professionals to optimize patient outcomes.
- Respond promptly to changes in patient conditions and emergencies, implementing appropriate interventions.

The required skills such as home health and hospice nursing are essential for delivering patient-centered care in non-hospital settings, allowing the RN to manage complex care needs effectively. Direct patient care and wound care skills enable the nurse to perform hands-on clinical interventions that promote healing and comfort.
