Position Overview:
INNOVATIVE WOUND CARE is seeking a Mobile Wound Care Provider (FNP/MD/DO)to join our growing team. This role offers the unique opportunity to provide hands-on, high-quality wound care in a flexible, autonomous environment . You’ll be integral to improving patient outcomes and expanding access to specialty wound care services in our community.

Location: Phoenix Metro Area (mobile)
Employment Type: Part-Time / Full-Time
Schedule: Flexible hours with patient-centered scheduling

Key Responsibilities:

• Perform comprehensive wound assessments and develop tailored treatment plans for chronic and acute wounds.
• Provide advanced wound care, including sharp debridement, infection management, dressing selection, and patient education.
• Collaborate with patients, families, and multidisciplinary healthcare teams to ensure optimal care and healing.
• Travel to patient locations (homes, assisted living, and SNFs) to provide mobile care.
• Maintain detailed and accurate documentation in the EMR system in a timely manner.
• Educate patients and caregivers on wound care management, prevention, and ongoing treatment goals.

Qualifications:

• Active and unrestricted FNP, MD, or DO license in AZ.
• Previous experience in wound care preferred; wound care certification (WCC, CWCN, or equivalent) a plus.
• Valid driver’s license, reliable transportation, and willingness to travel within the assigned region.
• Strong interpersonal and clinical skills with a commitment to compassionate, patient-focused care.
• Proficiency with EMR systems and clinical workflows.
