We’re currently seeking a Hospice Licensed Practical Nurse (LPN) who is compassionate , self-motivated , and ready to make a meaningful difference in the lives of patients and families in the Nogales, AZ area.

What You’ll Do:
- Provide supportive care based on patient and family needs in alignment with physician orders and care plans.
- Collaborate with the Nurse Case Manager to deliver personalized care.
- Communicate with physicians, staff, and other agencies to ensure the best use of resources and care coordination.
- Assist with assessments, data collection, and ongoing support for physical, emotional, social, and spiritual factors impacting patient health.
What We’re Looking For: Licensure: Current LPN/LVN license in Arizona. Driver’s License: Insured and reliable transportation required.❤️ BLS Certification: Active or willing to obtain. Skills: Strong communication, organization, and emotional resilience to provide excellent patient care. Extras:
- Arizona Class 1 Fingerprint Clearance Card (or ability to obtain).
- Bilingual (English/Spanish) required
- Home Health experience is a plus.
Qualifications:
- Ability to handle emotional stress and respect individual lifestyles.
- Availability for some after-hours and weekend coverage.
