PM Shift Certified Caregiver
The primary purpose of this position is to prepare and administer medications and provide assigned residents with routine daily nursing care and services in accordance with the assessment and care plan.
Essential Job Functions Duties and Responsibilities
Medication and Personal Nursing Functions
- Prepare and administer medications according to health care provider instructions and within the scope of the license.
- Maintain accurate documentation of medication administration, medication effectiveness and any noted adverse actions or side effects, in accordance with established policy and procedures, standards of care and applicable state/federal regulations.
- Accurately and safely manage, monitor and store medications in accordance with established policies and procedures and applicable state/federal regulations.
- Maintain cleanliness and organization of medication carts and any equipment and supplies. Immediately report any equipment or supply issues to management.
- Assure that medications are timely re-ordered and procedures are followed when medications are delivered.
- Attend and participate in in-services as required by federal, state and company requirements.
- Perform simple procedures as an extension of therapy services under the direction and supervision of the nurse, e.g. range of motion exercises, assistance in ambulation or exercises.
- Performs personal care activities including bathing, grooming, dressing, toileting, skin care/nail care, oral hygiene, shaving, dressing, and others as directed/requested.
- Assists in dining rooms as assigned to provide resident care and monitoring.
- Initiates and participates in leisure activities provided for residents. Encourages residents to socialize and participate in planned activities or programs and to develop friendships with other residents.
- Observes and documents the health and emotional condition of each resident, promptly reporting all changes to the nurse. Document significant events that occur during shift.
- Responds promptly, courteously and appropriately to resident requests for assistance and emergency pull cords. Complete proper paperwork.
- Promotes personal safety and a safe environment for residents by observing infection control, blood borne pathogens, fire safety, hazardous materials, and other safety practices, following facility guidelines, and reporting unsafe situation to the nurse.
- Assist residents in preparing for appointments and social programs.
- Assist with moving and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
- Assist residents to walk with or without self-help devices as instructed.
- Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
- Provide special resident services offered by the facility.
- Maintain a work environment that is clean and presents a professional image.
- Communicates with residents, visitors, staff and others in a courteous, clear and accurate manner and properly seek clarification or refer questions to nurse when appropriate.
- Follow established safety precautions in the performance of all duties.
- Observe resident confidentiality policies and procedures and provide and promote residents’ rights.
- Perform other duties as assigned by Resident Care Coordinator and other supervisors and team members.
- Maintain an acceptable attendance record.
- Learn and demonstrate Platinum Service standards.
Required Knowledge, Skills and Abilities
Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism. Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be able recognize the needs of and to work with ill, disabled, elderly, emotionally upset, and potentially hostile people within the facility. Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families. Must be dependable. Must be alert to surroundings and recognize and immediately report unsafe conditions.
Education and Experience
Prefer 6 months or more in home care or assisted living.
Licensing/Certifications
Must be certified in Basic Life Support and First Aid.

Essential functions are carried out in a variety of positions including walking, standing and sitting. This position requires manual dexterity to handle instruments and other equipment and the ability to observe and report changes in a resident’s condition to the nurse. The physical demands below reflect that employee must be able to push medication carts and provide assistance to residents. To meet residents’ needs, all positions require the ability to move freely through the building. Other physical demands are required as described below.
