Part-Time or PRN OT Home Health / Occupational Therapist

Job Summary:

• Assume responsibility for the ongoing assessment and development of individualized care plans.
• Provide ordered occupational therapy services and patient education.
• Evaluate functional status and home environment for hazards or barriers to independent living.
• Develop treatment programs and establish goals for improved function.
• Maintain appropriate clinical records and reports.
• Participate in quality assessment and performance improvement programs.
• Identify patient and family needs for additional home health services and make appropriate referrals.
• Instruct and supervise Certified Occupational Therapy Assistants.

Job Requirements:

• Must be licensed in the location where the Home Health Agency (HHA) is located.
• Graduated from an OT program accredited by ACOTE of AOTA.
• Eligible for or have taken the OT certification examination administered by NBCOT.
• Two years of clinical experience as an occupational therapist.
• Demonstrate good verbal and written communication, and organization skills.
• Possess and maintain current CPR Certification.
• Must be a licensed driver with a reliable insured automobile.
