At Brighter Days Home Health , we understand that healing involves more than medical care alone. We are seeking a compassionate and dependable Medical Social Worker (MSW) to join our growing home health team on a PRN basis .

This role is ideal for someone who is passionate about helping patients and families navigate difficult life transitions while maintaining dignity, independence, and access to essential resources.

What You’ll Do:

• Complete psychosocial assessments in patients’ homes.
• Assist patients and families with emotional support, coping strategies, and community resources.
• Help coordinate financial, social, and community services.
• Provide counseling and support related to illness, disability, adjustment, and long-term planning.
• Collaborate closely with nurses, therapists, physicians, and interdisciplinary team members.
• Participate in discharge planning and case conferences as needed.
• Maintain timely and accurate documentation in compliance with agency policies and Medicare regulations

Qualifications:

• Master’s Degree in Social Work (MSW) from an accredited program.
• Current Arizona licensure/certification as required.
• Minimum of one year healthcare experience preferred.
• Home health experience a plus, but not required.
• Excellent communication and interpersonal skills.
• Strong organizational and documentation abilities.
• Valid driver’s license, reliable transportation, and auto insurance.
