Your Responsibilities

• Conduct in-home patient assessments and develop individualized care plans

• Administer medications, wound care, and treatments as prescribed

• Educate patients and families on disease management, safety, and independence

• Monitor patient progress and adjust care plans as needed

• Supervise LPNs and Home Health Aides to ensure quality patient care

• Collaborate with an interdisciplinary team to coordinate comprehensive patient care

Qualifications

• Active Arizona Registered Nurse (RN) license in good standing

• Minimum of one year of nursing experience preferred

  • Home health, hospice, rehab, or critical care experience is a plus

• New graduates encouraged to apply

• Current CPR certification

• Valid driver’s license and reliable transportation

• Strong communication, organizational, and interpersonal skills
