What You'll Do

• Patient Assessment: Conduct comprehensive in-home evaluations to identify patient needs.

• Care Planning: Design and implement individualized care plans tailored to each situation.

• Team Collaboration: Lead and coordinate with care teams to ensure the best outcomes.

• Patient & Family Education: Provide clear guidance to help patients and families make informed decisions.

• Clinical Documentation: Maintain accurate records and share updates with primary physicians.

What We're Looking For

• Active Arizona RN license

• At least one year of home health experience (OASIS knowledge preferred)

• Reliable transportation and valid Arizona driver's license

• Current CPR certification and Arizona Fingerprint Card

• Bilingual (Spanish/English) skills highly preferred

• Associate's Degree in Nursing (or higher)
