• Already have established, active relationships with hospital discharge planners, case managers, hospitalists, or social workers in the Mohave County / Tri-State area who have sent you referrals in the past 24 months.
• Willing and able to do daily in-person visits to Western Arizona Regional Medical Center, Valley View Medical Center, Kingman Regional, Havasu Regional, Colorado River SNFs, etc. (100 % field-based – no remote/hybrid).
• Own a reliable vehicle + clean driving record (this is rural territory – daily driving is required).

The Role in One Sentence:

Use your existing relationships with discharge planners and case managers at WARMC, Valley View, Kingman Regional, Havasu Regional, and local SNFs to drive patient referrals to our in-home physical, occupational, and speech therapy services.

Must-Have Requirements (Non-Negotiable):

• 2+ years of successful healthcare referral/liaison experience specifically in Mohave County or the Tri-State market
• Current, active relationships that have sent you referrals in the past 24 months at facilities such as:
  Western Arizona Regional Medical Center (Bullhead City), Valley View Medical Center (Fort Mohave), Kingman Regional Medical Center, Havasu Regional Medical Center, or local skilled nursing facilities.
• Live in Mohave County or the immediate area today (remote and relocation candidates will NOT be considered).
• Reliable vehicle + clean driving record (mileage reimbursed at IRS rate – expect 15k–25k miles/year).

Compensation:

• Base salary $70,000 – $75,000 (higher end for proven local book).
• Uncapped commissions – proven reps in this territory clear $140k–$180k+.
• Full benefits (health, dental, vision, 401k match, PTO).
• Mileage + phone reimbursement.
