Hello Dr, Hope you are doing well! The hospital is looking for NP - Hospitalist Nurse Practitioner in Goodyear, AZ Please review below details and let me know the best level of interest: JOB DESCRIPTION: SUBMISSION REQUIREMENTS- Must have clean background and malpractice
- Must have ATLS
- Must have AZ license and DEA
- Must have ACNP or certification
- 1st assist & trauma experience preferred
- At least 1 year of experience in Trauma/ED and ICU Submission Requirements
- All candidates must have met the minimum shift availability listed on the job.
- CV (Curriculum Vitae) must have all dates in MM/YY to MM/YY format
- Work History must be in Sequential order with most resent-on top
- Any Gaps over 30 days must be explained on the CV
- EPA (Envision Pre-Application) (Please use the attached EPA. It is also in Trio under your Name & then click on Help)
- All Licenses, Boards and Certs with expiration dates must be listed on CV
- Anyone with malpractice must provide a NPDB
- Anyone with legal issues must provide supporting documents
- 1st date would they be available?
- How many shifts would they be able to do per month?
- What month is their schedule completely open?
- Any shift preferences (Day, Night, Blocksetc.)?
- Any dates in the next 3-6 months they cannot work?
- Any Holidays in the next 3-6 months would they be open to work.?
- 200-bed facility
- Level I Trauma Centre (State), Level II (ACS)
- ED Volume: 55k; 33 beds
- 14 days of EGS call per month
- Inpatient census: 20/day
- 20 ICU beds, additional 10 medical ICU beds
- ATLS
- BLS
- Board Certified
- Arizona
