Your Role as a RN:

We are seeking aRegistered Nurse (RN) Case Managerto provide skilled nursing care to our home health patients. This position is available asfull-time or part-time, with flexible scheduling.

Youll work closely with an interdisciplinary team to:

• Conductadmission and OASIS assessments
• Performhead-to-toe and wound evaluations
• Create and implement individualizedplans of care
• Providefollow-up skilled nursing visits
• Educate patients and caregivers
• Coordinate care with MDs, CNAs, PT/OT, and family members
• Attendweekly team meetingsand contribute to care planning

Pay and Benefits

• $60 to $110 per visit

Qualifications:

• CurrentArizona RN license
  
• Reliabletransportation
• Excellentcommunicationandinterpersonal skills

Preferred Qualifications:

• You have prior home health experience
• You have experience charting in Matrix
