Weekday Schedule: Monday - Thurs 5:00 PM -8:00 AM each day

Your Impact

As our On-Call Hospice, you will:

• Respond quickly and compassionately to patient and family needs during on-call hours

• Deliver skilled nursing care, symptom management, and emotional support

• Partner with RNs, physicians, and the interdisciplinary team to provide seamless care

• Guide families through the hospice journey with clear education and reassurance

• Ensure continuity of care with accurate documentation and effective handoffs

What You’ll Bring

• Active LPN license in good standing (required)

• Minimum 1 year of hospice or home health experience preferred

• Strong clinical judgment and problem-solving ability

• Excellent communication and interpersonal skills

• A commitment to patient-centered, compassionate care

• Reliable transportation and valid driver’s license
