We’re seeking a Licensed Practical Nurse (LPN) to join us in Green Valley and Sahuarita, AZ!

What You’ll Do:

• Patient & Family Support: Identify and respond to patient and family needs, following physician orders and care plans.
• Collaborate with Care Team: Work closely with Nurse Case Managers, the Program Director, and Medical Director to provide holistic, interdisciplinary care.
• Communicate & Coordinate: Ensure open communication with team members, physicians, and external agencies for comprehensive patient care.
• Data Collection: Gather information on physical, emotional, social, and spiritual factors affecting patients and their families.

What We’re Looking For:

• Current LPN/LVN license in Arizona
• Excellent communication and organizational skills
• Driver’s license and insured vehicle
• Current BLS certification and Fingerprint Clearance Card (or willingness to obtain)
• Compassionate nursing skills with respect for individual lifestyles
• Home Health experience and bilingual (English/Spanish) preferred!
