Role specifics: You’ll serve as the critical pharmacist on the care team, validating prescriptions, ensuring accurate dispensing, and providing patient counseling on medication use, interactions, and adherence. You’ll conduct medication therapy management (MTM) sessions, review high‑risk medications, monitor adverse effects, and collaborate with prescribers to optimize regimens.
