Overview of the Position

Job Title: Primary Care Provider (NP/PA)

Location: Prescott, AZ

Hours & Schedule: Mon - Fri (0800 - 1700)

Travel Required: No travel required

Salary / Hourly Rate: $120k - $150k / year

Bonus Offered: RVU

Job Summary

We are seeking an experienced Primary Care Provider to deliver high-quality, patient-centered care. The role involves managing well visits, chronic disease treatment, and preventive care. Providers will work within our structured care flows, using EHR systems efficiently, and focusing on patient outcomes in a collaborative clinic environment.

Job Duties & Responsibilities

• Conduct well visits, including annual physicals and wellness exams
• Address same-day patient needs (e.g., fever, UTI, allergies)
• Manage chronic conditions such as diabetes, hypertension, and COPD
• Promote preventive care, healthy living, and disease prevention
• Order diagnostic testing and labs as needed
• Adhere to established care flows for treatment
• Maintain proficiency with EHR systems and ensure timely note completion
• See 10-15 patients per day

Prerequisites / License & Certification Requirements

• Arizona Licensed Provider (NP/PA)
• NP/PA Program Completed in USA (Excluding Puerto Rico)
• Active DEA License (AZ) without restrictions
• No history of malpractice
• Eligible for credentialing with Medicare, Medicaid, and commercial insurance providers
