JOB DESCRIPTION SUMMARY:

The Physical / Occupational Therapist (PT/OT) is responsible for evaluating, planning, and implementing individualized therapy programs to help clients improve mobility, restore function, and maintain independence in daily living activities. This role focuses on enhancing quality of life through therapeutic interventions, education, and collaboration with the care team.

JOB FUNCTIONS/RESPONSIBILITIES:

Responsibilities of the Home Health Aide include, but are not limited to the following:

1. Conduct comprehensive physical and/or occupational therapy assessments
2. Develop and implement individualized treatment plans based on client goals
3. Provide therapeutic exercises, functional training, and rehabilitation techniques
4. Assist clients in improving mobility, strength, coordination, and daily living skills
5. Train clients in the use of adaptive equipment and assistive devices
6. Educate clients and caregivers on therapy plans, exercises, and safety practices
7. Monitor and document client progress and modify treatment plans as needed
8. Collaborate with physicians, nurses, caregivers, and other healthcare professionals
9. Maintain accurate and timely clinical documentation in compliance with regulations
10. Promote client independence and safety in home or care settings
11. Participate in care planning meetings and case conferences

QUALIFICATIONS

• Degree in Physical Therapy or Occupational Therapy from an accredited program
• Current state licensure as a Physical Therapist and/or Occupational Therapist
• Previous clinical or home care experience preferred
• Strong understanding of rehabilitation principles and practices
• CPR/First Aid certification (as required)
• Ability to travel daily including weekends, nights and holidays.
• Understands regulatory compliance
• Valid Level 1 Fingerprint card
• Empathy for the needs of the client
• Demonstrate effective oral and written communication skills
• Ability to express spoken and/or written ideas in English
• Treats clients, staff and the public with courtesy, respect and presents a positive public image.
• Works as a team member
• Ensures confidentiality and security of the client’s medical information.

ENVIRONMENT, PERSONAL PROTECTIVE EQUIPMENT OR PHYSICAL DEMANDS

Working Conditions: Contact with clients under a wide variety of circumstances. Care provided in a client's living environment with varying situations or in an institution such as a hospital. May be required to respond in an emergency situation.

Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.

Personal Protective Equipment: May include gloves, mask, eye protection, and disposable outer covering.

Physical Requirements: Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift up to 50 pounds.
