Respite Case Coordinator / Case Manager

Position Summary

The Respite Case Coordinator supports youth participating in the weekend Therapeutic Respite Program by coordinating care, scheduling services, and providing program and administrative support. This role works closely with parents/guardians, clinical teams, and internal staff to ensure a safe, structured, and therapeutic respite environment. The Respite Case Coordinator plays a key role in helping families receive temporary relief while ensuring youth receive consistent, high-quality services.

Location: Gilbert / Phoenix

Schedule: Tuesday through Saturday 8:00am - 5:00m

Essential Duties & Responsibilities

1. Client Scheduling & Communication
2. Coordinate, schedule, and confirm weekend respite services for enrolled youth.
3. Serve as a primary point of contact for parents/guardians regarding schedules, expectations, and required documentation.
4. Provide clear communication related to upcoming respite stays, check-in/check-out procedures, and program requirements.
5. Track and follow up on missing or incomplete documentation needed for participation.
6. Communicate scheduling conflicts, family concerns, or documentation issues to the Lead in a timely manner.
7. Managing and obtaining up -to - date client assessments
8. Completing accurate notes and billing activities
9. Program support by providing occasional transportation, client check-in and activity support
10. Ensuring all agency polices and Adminstration Code reuquirments and payer documentation standards

Minimum Qualifications

1. High school diploma or GED required; Associate’s or Bachelor’s degree in Behavioral Health or a related field preferred.
2. At least one (1) year of behavioral health experience preferred, especially with youth/adolescents.
3. Strong communication and organizational skills.
4. Ability to work weekends consistently.
5. Valid driver’s license, reliable transportation, and ability to pass required background checks.

Work Environment

1. Weekend-based schedule supporting therapeutic respite programming.
2. Combination of office-based, program-based, and community-based work.
3. May include transporting youth and supporting on-site program activities.

This position plays an essential role in supporting youth and families through compassionate coordination, organization, and teamwork.
