  • Address barriers to care, including scheduling challenges, insurance concerns, and patient education needs.
  • Educate patients on gastrointestinal conditions, treatment plans, medication adherence, dietary considerations, and follow-up care.
  • Follow up on diagnostic tests and communicate results and next steps to patients.
  • Provide clinical support, counseling, and education; escalate urgent clinical situations to providers as needed.
  • Collaborate with the GI team to develop and implement care plans focused on improving patient outcomes.
  • Ensure thorough documentation of patient encounters, laboratory results, and treatment plans in the electronic medical record (EMR).
  • Work with the multidisciplinary team to reduce readmissions and address chronic disease management and social or environmental factors impacting patient care.
  • Assist GI providers in addressing any gaps in care by coordinating follow-ups, procedures, and referrals.
  • 5% of responsibilities will involve direct clinical duties, including ordering labs, reviewing patient records, and supporting procedures as needed.
